
Eyecare Insurance and Billing
Eyecare Associates works with a number of insurance companies that offer vision care. If you don’t have insurance, we partner with Care Credit which can help you finance your eye care needs.
To see if we work with your insurance carrier or vision care plan, please call your insurance company or our office for assistance. Our friendly and informed staff will be happy to answer any questions you may have. We accept:
- VSP
- Oregon Health Plan
- Health Net
- Moda
- Medicare
And many others.
We do not accept Eyemed discount plans, Davis Vision, or FEP Blue Vision.
Patients With Insurance
You are responsible for deductibles, co-pays, non-covered services (such as the Optomap which is $39 and/or contact lens fees), co-insurance, and items considered “not medically necessary” by your insurance company.
Please pay co-payments and co-insurance amounts as services are rendered. The remaining balance should be taken care of within one (1) month of notice from your insurance company unless other arrangements are made with our billing department. If you or your insurance carrier make a payment that exceeds your balance, reimbursement will be gladly remitted.
If payment cannot be made at your visit, please notify our billing department to make other arrangements.
Medicare
Our office will submit your claim to Medicare and your secondary insurance. You are responsible for deductibles, copays, and any non-covered services, such as the refraction ($20) and the Optomap ($39). If you do not have a secondary insurance, you will be billed for 20% of the exam.
Patients Without Insurance (Private Pay)
We ask that you please pay for your care in full after each visit. If payment cannot be made, please check into applying for Care Credit before or during your visit.
